Because nearly every published list is the American one. Google localises category names, and for a fair share of healthcare categories the UK name is different. Sometimes it is only spelling, but often it is a different word: what Americans call a Physical therapist, Google calls a Physiotherapist in the UK, and a Podiatrist is a Chiropodist. Your primary category is the single strongest signal you send Google about what your business is, and it carries far more weight than the rest. Secondary categories cover the other things you genuinely do. Pick the primary that most precisely describes your main service, then add secondaries only where you really offer that service.

Only as many as are genuinely accurate. Categories you do not actually serve dilute the primary signal and surface you for searches you cannot fulfil, which costs you relevance and wastes enquiries. A focused clinic with three accurate categories will usually outperform one with ten loose ones.

Google's healthcare taxonomy has real gaps and there is no way to request a new category. The usual answer is to choose the closest accurate parent category as your primary, then let your services, business description and website carry the specificity Google's list cannot.
